What You Need To Know

  • Home Health is clinical, skilled care provided by licensed professionals (like nurses and therapists) and typically requires a doctor’s order. It includes things like medication management, wound care, and physical therapy.

    Home Care is non-medical, supportive care provided by aides. It helps with activities of daily living such as bathing, grooming, meal preparation, light housekeeping, and companionship.

  • Hospice care is compassionate end-of-life support for individuals with a life-limiting illness when curative treatment is no longer effective. It focuses on comfort, dignity, and emotional support for both the patient and their loved ones. Services may include pain and symptom management, spiritual care, bereavement support, and caregiver guidance.

  • Our services are typically covered by:

    • Medicare and Medicare Advantage Plans

    • Medicaid (for qualified individuals)

    • Private insurance (depending on plan benefits)

    • Private pay (in rare cases where services are requested without a referral or qualifying medical need)

    We’re happy to verify your benefits and walk you through your options.

  • For home health and hospice, yes—a physician’s referral is required. For personal care services, you may contact us directly to schedule an assessment and discuss your needs.

  • Simply give us a call or have your physician send a referral. Our team will schedule an in-home or phone assessment, talk through your goals, and develop a personalized plan of care based on your needs.
