Service Planning and Coordination
After the consultation and assessment are complete, the care manager can begin the work of developing a plan to address all needs identified. The plan may include companion care, community services such as transportation and adult day services as well as involving family and friends in daily tasks where support is needed. The care manager and family meet to agree upon desired services and the care manager makes arrangements to bring them into the home.
