John and his young family live in Minnesota, half-way across the country from his mother and father, June and Martin, who call Blacksburg, Virginia home. June suddenly falls ill and requires hospitalization and follow-up rehabilitative services and Martin’s declining health and poor vision prevent him from serving as her caregiver. June needs additional help.

While John can spend some time with his parents to get them home from the hospital, arrange follow-up doctor visits and get them settled in at home, it becomes apparent that more help is needed to see his mother through her recovery. He must return home to Minnesota to his job and family. John calls on the services of Warm Hearth at Home. Before he returns home, John and his parents establish a relationship with the care manager. They then work together to conduct a full assessment and a plan of care that will meet all their needs. The care manager facilitates in-home companion care for June and Martin, transportation services to the doctor and grocery store, and help with paying the bills and minor repairs as needed. John returns home confident that his parents are in good hands and able to stay in their home until June recovers. He is reassured by the fact that the care manager will monitor and report to him on his parents’ progress and any other services that may be needed along the way.
